Water vapor-procedure to treat prostate enlargement.

As a highly innovative therapy for benign prostatic enlargement the German Medical Centre (GMC), Dubai offers the water vapor procedure

For this treatment, a thin needle is inserted into the prostate during an endoscopic cystoscopy. By this way 5ml water vapor is injected, that causes boiling of the tissue and finally shrinking of the prostate.

The duration of the procedure is less than half an hour, depending on the number of necessary injections (usually 2-12). Local anesthetics or slight general anesthesia is sufficient for good tolerance.

Since swelling of the prostate can occur for a while, we leave a small catheter for a few days in the bladder to ensure urine drainage. To reach the complete amount of prostate downsizing a couple of weeks are necessary.

The procedure is indicated for symptoms induced by an enlarged prostate, e.g.:

Reduced urinary flow,

Residual urine in the bladder after voiding or urinary retention,

Frequent voiding, urgency, urge incontinence,

Bleeding from the prostate gland,

Insufficient efficacy of medical treatment,

In the case of prostate cancer, symptoms by prostate enlargement can be treated, if surgical prostatectomy is not indicated.

Is not reliable effective and is therefore not recommended for very large glands (> 80 – 100ml) and if other reasons are predominant for the symptoms. Penile implants or artificial sphincters are contraindications.

The treatment is minimal-invasive and very gentle:

Needles are injected into the prostate under direct endoscopic vision; this ensures, that there is nearly no risk for sphincter damage and for stress incontinence.

Erectile function is typically not affected.

To protect the ejaculation specific position of water vapor application can be chosen.

Risk of bleeding is very low since very thin needles are inserted only a few millimeters. This enables the application while taking aspirin, coumadin or other anticoagulants and bleeding diathesis.

The burden by this procedure is very low, so it can be applied even for patients with severe general diseases who are not fit for other surgical procedures (e.g. transurethral resection).

Sometimes irritative symptoms, infection or very slight bleeding occurs after the treatment and are usually of short duration.

We would like to advise if this procedure is adequate therapy for specific patients and information about risks and side effects.

Prof. Kai Uwe Köhrmann, the consultant surgeon in the German Medical Centre, is the very first urologist authorized to perform treatment in Dubai.

He and a lot of other urologists in the world are convinced, that this innovation is ideal for many men with bothersome prostate enlargement, especially if other treatment options are not effective, contraindicated or not preferred by the patient.
